Basically, instead of making the lobe taller, they make everything else smaller. You then use thicker shims or adjust your rockers (depending on what type of valvetrain you have) to make up for the difference. The reason why custom ground billet cams are better is because reground cams force your valves to accelerate and decelerate at a faster rate than billet cams. This can cause rapid wear of the valve seats and is hard on your valve springs.
